(DLSMHSI) is a Lasallian Catholic institution dedicated to the holistic
formation of competent, compassionate, and ethical professionals in the medical
and allied health sciences. Guided by its mission of nurturing life,
DLSMHSI integrates education, healthcare, and research within a God-centered
and service-oriented environment that upholds the Lasallian core values of Faith,

As a full-complement institution, DLSMHSI operates
through three core components: Academic Services, which ensure the delivery of
quality medical and health sciences education; the De La Salle University
Medical Center (DLSUMC), which provides accessible and excellent patient care;
and the De La Salle Angelo King Medical Research Center (DLSAKMRC), which
advances health research and innovation. These components work synergistically
to embody the Institute’s mission and promote its integrated approach to education,
service, and research.

2. Original Form 138 (4th year High School Report Card)
3. Photocopy of High School Diploma or Certificate of Graduation
Transferee Applicants
1. Photocopy of Birth Certificate
2. Original Transfer Credential/Honorable Dismissal from the previous school
3. Completed and signed Advising & Crediting Form from the DLSMHSI College secretary
Additional Requirements for Foreign applicants
1. Photocopy of Alien Certificate of Registration (ACR) for permanent residents, native born, children of diplomats, or holders of any valid visa and Certificate of Residence for Temporary Students (CRTS) (for foreign applicants).
2. English Proficiency Result (eg, TOEIC / IELTS, etc.) or from the Institute’s Language Training & Professional Development Unit (LTPDU) (for non-native English speakers).
3. Certificate of Completion for Improving Tagalog Communication Skills (Special Filipino Class) from the Institute’s Language Training & Professional Development Unit (LTPDU) (for Nursing applicants).
